What is Prostatitis?

Prostatitis is a condition that involves inflammation and swelling of the prostate gland. It is a relatively common issue, particularly among men under the age of 50, though it can occur at any stage of life.

Each year, prostatitis leads to a significant number of medical visits, with millions of men seeking urologic care for related symptoms. Because the condition can cause discomfort and urinary problems, proper evaluation and treatment are important for relieving symptoms and addressing the underlying cause.

What are the Symptoms of Prostatitis?

Common symptoms of prostatitis may include:

• Pain or discomfort during urination

• A burning sensation when urinating

• Increased urinary frequency, especially at night

• Blood in the urine (hematuria)

• Cloudy urine

• Pain during ejaculation

• Pain or discomfort in the area between the scrotum and rectum (perineum)

• Testicular or scrotal pain

• Flu-like symptoms, such as fever, chills, or body aches

Symptoms may appear suddenly or develop gradually, and they can be constant or come and go over time. In some cases, prostatitis may not cause noticeable symptoms. This form, known as asymptomatic prostatitis, is often discovered during evaluations for other conditions, such as during a routine prostate exam or other diagnostic testing.

Why Do I Have Prostatitis?

One possible cause of prostatitis is bacteria from the urinary tract entering the prostate gland. Bacteria responsible for certain sexually transmitted infections can also lead to prostate inflammation. In some cases, physical trauma to the pelvic or scrotal area or nerve damage following surgery may contribute to the condition.

However, for many men diagnosed with prostatitis, a specific underlying cause cannot always be identified. When this occurs, treatment typically focuses on relieving symptoms and improving quality of life while monitoring the condition.

What Tests Can Diagnose Prostatitis?

Diagnosis of prostatitis usually begins with a physical examination and a review of your medical history and symptoms. Your specialist may order urine and blood tests to check for signs of infection or other underlying conditions that could be contributing to your symptoms.

In some cases, your urologist may perform a prostate massage to collect prostate fluid for laboratory testing. Imaging studies—such as a CT scan or prostate ultrasound—may also be recommended to evaluate the prostate and surrounding structures.

While the exact cause of prostatitis is not always identified, these evaluations help rule out conditions such as bacterial infections or nerve-related issues and allow your provider to develop the most appropriate treatment plan.

How Do You Treat Prostatitis?

Treatment for prostatitis depends on the underlying cause and severity of symptoms. If a bacterial infection is identified, antibiotics are often the primary treatment. The specific medication prescribed will depend on the type of bacteria involved. Most patients take antibiotics orally, although severe infections may require intravenous (IV) antibiotics.

If urination is painful or difficult, medications such as alpha blockers may be recommended. These medications help relax the muscles around the bladder neck and prostate, which can reduce discomfort and improve urinary flow.

In addition to medical treatment, certain lifestyle adjustments may help relieve symptoms. Warm baths, avoiding alcohol and other bladder irritants, and staying well hydrated may help reduce inflammation and improve comfort. A urologist can evaluate your symptoms and recommend a treatment plan tailored to your specific condition.
