Chronic Care Management (CCM) services are designed to support patients who have ongoing health conditions that require continuous monitoring and coordination of care. These services provide patients with access to a dedicated care team that helps manage health needs, improve communication between providers, and ensure that treatment plans are followed effectively.

Through CCM services, patients may receive:

  • Ongoing health assessments and monitoring from the care team

  • Personalized care plans designed to support preventive services and long-term health management

  • Medication management and oversight

  • Coordination of care with other physicians or healthcare providers when needed

  • Access to the Chronic Care Management team for questions and support

As part of these services, the care team will explain how the program works, provide updates or adjustments to your care plan when necessary, and notify you if CCM services are ever discontinued.

Participation in CCM services allows the practice to coordinate your care with other providers, hospitals, or caregivers as needed, including secure electronic communication related to your treatment. These services may be billed to Medicare or other applicable insurance plans. Patients may be responsible for certain charges depending on their insurance coverage, deductible status, or co-insurance requirements.
